A year after President Trump announced a plan to lower Medicaid drug prices, the initiative remains largely unfulfilled. On September 30, 2022, Trump and Pfizer CEO Albert Bourla revealed a voluntary agreement aimed at reducing prescription drug costs for Medicaid, the government program that serves 66 million low-income and disabled Americans. However, details about which drugs will see price reductions are still unclear, and the program's implementation is ongoing.

The push to lower drug prices comes as Medicaid spending on prescription drugs has surged. According to the Kaiser Family Foundation, Medicaid's net spending on medications increased by 46% from 2019 to 2024, costing taxpayers approximately $46 billion in 2024 alone. Trump touted the agreements, referring to them as "most favored nation" deals, claiming they would align U.S. drug prices with those in other wealthy countries. Despite these claims, the agreements with 26 pharmaceutical companies have not been publicly disclosed, leaving many questions unanswered.

Trump's announcement included the launch of TrumpRx, a website designed to help consumers find discounts on medications. However, the site has faced criticism for its limited selection, with many users finding better prices through their health insurance plans instead. Trump's promise that all Pfizer medications would be offered to Medicaid at these favored prices has yet to materialize, and participation from other drug companies remains uncertain.

The lack of transparency and operational challenges could hinder the effectiveness of the program. As it stands, the pilot relies on voluntary participation from drug companies and individual states, which may limit the number of discounted drugs available to patients. Without clearer guidelines or commitments from the pharmaceutical industry, the promise of reduced Medicaid drug prices remains in limbo, raising concerns about the administration's ability to deliver on its commitments.
